Is it possible to pre plan subs? like say after 60min new players come onto the field replacing tired players.

No to put it simply. It's one of the bonuses you get for being online during matches. Sucks for awkwardly timed matches and people in vastly different timezones but it can't be done. I guess they have to have some kind of bonus for being online

Open up your strategy building. You can then drag and drop your players to change your starting lineup. You can sub during a match the same way.
